Issue
You did not enable "Push Notifications" from Duo on your smartphone after downloading the app and are having an issue logging in on your desktop.
PLEASE NOTE: Once you have opted into DUO there is NO way to opt out or have it removed from your account.
Resolution
To Resolve using an iPhone
1. Go to the "Settings" option in your phone.
2. Press the "Notifications" option
3. Scroll down to "Duo Mobile", press it
4. At the top, make sure "Allow Notification" is toggled on
To Resolve using an Android:
1. Open "Settings"
2. Select "Apps"
3. Scroll to find "Duo Mobile", select it
4. View "App Info Page" and select "Notifications"
5. Make sure "Notifications" are toggled on
